Primary Household Contact Information
The primary household contact section will be used as the primary contact for OSL. The Primary Contact does not have to be the head of household - but is the individual whose address and mobile number will receive email and text notications as appropriate. We will be making more use of email and mobile text as an alternative to traditional mail to send out communications including newletters, announcements, notices and quarterly contribution reports based upon the preferences you indicate. Communication Preferences
Our Saviour's will be communicating with its membership on a variety of topics. It many cases, in addition to using postal mail services, we can now utilize email for communications such as your contribution statements or newletters. Additionally you may option to choose neither. Please select your preferences for the following communications. If you do choose email it will be sent to the address listed in the Head of Household Contact Section.
Household Member Contact Info
We have included sections for each additional members of the household. If household members are at a different address please create a household contact by using an additional registration. Fill out only the information appropriate and you wish to have available to the church for contacts. We have provided 6 additional slots - if you need more please use an additional form or contact the church office (906.774.2733)
